Fire in a Fire Phase is done alternately starting with the player that won initiative.
Does the effect of fire take place immediately? i.e. if I'm the player with initiative, I fire first and destroy a stand, does that stand get to fire back before it's removed?

Yes, the results take effect immediately. Check the step by step standard fire procedures on page 10. The last step G ends with ..."and apply the results".
This makes things rather tense in the standard fire phases as you and your opponent decide which of your groups to fire first. Modern warfare is lethal and the army that has the initiative has a distinct advantage.
